Community Nursing

The prime aim of the national disability insurance scheme is to provide assistance to those who are in need. Perfect Care is registered with the NDIS to provide services to disabled people in the Melbourne region. Community nursing care is one of the services that we provide to you or your loved ones.

What is community nursing care?

​

Community nursing care services are to provide assistance with your clinical and personal needs. Depending on the kind of disability you have, we will help you live an independent and healthy lifestyle

​

•This service will be delivered to eligible NDIS participants and involves services like general nursing, wound management, assistance with chronic conditions, or hospital-type care outside of the hospital. We provide this service to prevent disease, maintain health, and assist with existing health problems; for that, we work directly with you and your family members.

​

•Community service also works as an interpretative bridge between different community service providers. So that you do not have to worry about anything, our support worker can be your voice. This allows you to focus more on your recovery because our support staff will look at your nursing care.

Community Nurse Roles and Responsibilities

•The nurse assigned to you will have several responsibilities and roles according to your medical condition. Here is a list of those

•Care after a hospital stay

•Developing treatment plants

•Case management

•Medication management

•Wound management

•Stoma management

•Diabetes management

•Respiratory support

•Overnight nursing care

•Personal care assistance

All of the above responsibilities will be delivered while closely communicating with the doctors and other medical staff to provide you with the best possible service. These responsibilities will be delivered by expert nurses, who will have a number of years of experience.

Community nursing staff work

•The community nursing service will begin by assessing you and deciding what kind of assistance you will need. After that, our staff will plan your recovery from the start.

•Their first priority will be taking care of you, and then they will focus on how to make you self-sufficient. Our trained support staff will guide you through the basic tenets of personal care.

​

•They will monitor your day-to-day progress and share that data with official medical staff to decide the next step of recovery if any special care is needed or if you need to introduce activities.

​

•In this process, you will learn a lot of things because our end goal is to get you to a point where you can become more independent in your life. To avail of these services, you have to contact us.
